Eligibility
In order to qualify for Home Health Services you must:

Be confined to your home
Be under the care of a licensed physician
Have someone who is willing to assist with your care
Require intermittent skilled nursing or rehabilitation services

Monday through Friday

Payment
Medicare, TENN-CARE, private health insurance, Workman's Comp, auto insurance or private funds may pay for all or part of the Home Health charges. Our staff will work with you to determine what type of financial assistance you may be eligible for. All paperwork will be handled by the agency.

A Note to Physicians
Our goal is to assist you in caring for your patients. To obtain our services, call (423) 636-0700

A registered nurse will promptly visit your patients and notify you of any significant findings.

You will continually be kept informed of the progress of the patient, and the staff of Takoma Home Health will work closely with you to establish an individualized program.
