Joining many of the top hospitals in the country, Takoma’s hospitalists now “round” on its patients with a multi-disciplinary team on weekdays.
“We actually go as a team into the patient’s room and have a real-time group discussion about the plan of care with our patients and their families,” Tammy Albright, the hospital’s chief nursing officer, said.
The team is comprised of physicians, nurses, pharmacists, social services, therapists, case managers, and other providers as needed.
“Multi-disciplinary rounding puts the patient at the center of care,” Albright said. “The patient is evaluated by several different disciplines at one time. The patient is then educated and involved in the final plan of care. We have found this to provide more effective, coordinated care; potentially shorten lengths of stay; and increase patient satisfaction.”
